We have an SSL certificate and have the capability of using collected information securely over the internet with 128-bit SSL encryption security but do not have any quoting or application forms on this site that collect personal information.  We do have a Contact Us form on the site, and if you utilize that form to contact us, any information you provide comes to us in the fashion mentioned in the previous sentence.

We NEVER sell or rent your contact information, EVER. 

We share your information ONLY with MPowering Benefits, LLC and/or Sedera Health and ASI, if you are enrolling in a group medical cost sharing plan; with American Memorial (Assurant) if you are applying for a Pre-Need Funeral Arrangements Plan; or with individual health sharing organizations if you are enrolling in an individual healthsharing plan or program, for purposes of enrollment; determining program eligibility; determining monthly share contributions; lump sum or premium payments for a Pre-Need Funeral Arrangements Plan; determining if any pre-existing conditions are applicable; or otherwise administering the plans or programs, including the health and wellness programs provided by My Academy of Health Excellence and marketed through MPowering Benefits.

We follow HIPAA and HITECH guidelines and requirements. We send and receive replies containing Protected Health Information (PHI) and Personally Identifiable Information (PII) using secure email or attended fax.  

We protect the confidentiality and security of your personal and health information.  We maintain physical, electronic and procedural safeguards to protect your personal and health information. 

We collect Personally Identifiable (PII) and Personal Health Information (PHI) only to the extent necessary to determine your eligibility for participation in the program and to complete or help complete your application, depending on the method used for processing your application. 

We continue to evaluate our efforts to protect your personal and health information and make every effort to keep this information accurate and up to date.  If your personal or health information changes, or if you otherwise want to correct, update or delete your contact information held by us, you may contact us by email at by mail at MPowering Benefits LLC, c/o Paul Cholak,1761 W. Please note that once your application has been submitted to the applicable provider you may have to contact MPowering Benefits, Sedera Health, ASI, American Memorial (Assurant), My Academy of Health Excellence or another program provider directly to update your application, PHI, or PII, or to enroll separately in a separate, specific program if that is required.

Information collection, use and sharing. Control Your Health Care Costs, LLC (dba Control Your Healthcare Costs), My Academy of Health Excellence, MPowering Benefits, Sedera Health, and ASI, as applicable for group medical cost sharing plans; MPowering Benefits and American Memorial (Assurant) for Pre-Need Funeral Planning Arrangements; Control Your Health Care Costs (dba Control Your Healthcare Costs) and individual healthsharing organizations, as applicable for individual healthshare plans/programs; and My Academy of Health Excellence for health and wellness programs, are the sole owners of the information collected on this site.  We only have access to collect information that you voluntarily give us via email or other direct contact from you.  Unless you ask us not to, we may contact you via email or phone in the future to tell you about new products or services, send you a newsletter that contains information about healthsharing and health and wellness; information for "self-pay" patients; or changes to this privacy notice.

Applying for Group Medical Cost Sharing Plans, Individual Healthsharing Plans or Progams. a Pre-Need Funeral Arrangement Plan, or My Academy of Health Excellence Health and Wellness Programs. If you create an account and ask us to assist you in making application through My Academy of Health Excellence, MPowering Benefits, and/or Sedera Health and ASI websites, for group health medical cost sharing plans; to indivudal health sharing plans; to individual health and wellness programs, as applicable; or to My Academy of Health Excellence for health and wellness programs; or use a paper application to apply to American Memorial (Assurant) for a Pre-Need Funeral Arrangement Plan, we, My Academy of Health Excellence, MPowering Benefits, Sedera Health, ASI, American Memorial (Assurant) or individual health sharing organizations may ask you to provide personal, financial, and/or health information about you or one of your dependents. This information helps determine plan eligibility and collection of monthly share contributions;  lump sum payments or premiums, and membership/enrollment fees, as applicable for the particular product. We follow HIPAA and HITECH rules in collecting this information. 

Security of information. We send information containing personally identifiable information (PII) and/or protected health information (PHI) by secure encrypted email using the LuxSci system; utilize the My Academy of Health Excellence. MPowering Benefits, Sedera Health, ASI, or individual health sharing organizations' secure websites to collect information for making applications, or fax the information (using a coversheet) to an attended secure fax. Our computers are all encrypted. We take precautions to protect your information both online and offline. 

Wherever we collect sensitive information other than verbally (such as information about health conditions or payment information), that information is encrypted and transmitted to us in a secure way. You can verify this by looking for a closed lock icon at the bottom of your browser or looking for "https" at the beginning of the address of the web page.

While we use encryption to protect sensitive information transmitted online, we also protect your information offline.  Only those who need the information to perform a specific job are granted access to PII or PHI. The computers in which we store PII and PHI are kept in a secure environment.        

Providing you with a quote, processing your application, or administration of the program. We may use your personal information to get in touch with you when necessary to process your application, provide you with a quote for monthly share contributions, provide a quote for a Pre-Need Funeral Arrangement Plan. determine if any pre-existing condition requirements apply, and/or administration of any of the programs once you have enrolled. For example, we may call or email you throughout the application process to inform you of the status of your application and to seek additional information that is requested as part of the application. 

Privacy and security policy. We have developed and implemented a privacy and security policy.  

Cookies. We do not take applications directly on this website, but we do utilize applications that use cookies (small simple and benign text files temporarily stored on your computer) for the sole purpose of saving your information for you as you fill out appiicable application forms online. This is for your convenience so that your browser will remember your form entries if you decide to go to a previous page to check a previous answer. Once the application form has been completed, the cookie is deleted from your computer by your browser, and the applicable system deletes its copy of the cookie (called a "session"). 

The information gathered by the cookie is whatever information you put in the fields of the application form. Once you submit the form, the cookie is no longer needed, so it is deleted.      

THIS site does not use cookies.  However, you must accept cookies in order to fill out group medical cost sharing or individual healthsharing program application forms online from applicable medical cost sharing or invidiual health sharing providers, ASI, or to enroll in the Bronze, Gold, or Platinum Plans offered by the Academy of Health Excellence.
